The undersigned, exercising parental responsibility (or guardian) over the registered minor, declares that they have read the rules of GYM Prova and the safety rules of the activity, and accepts them also on behalf of the minor.
The parent declares that the minor is in good health and fit for sport, and undertakes to provide the required medical certificate and to promptly report any conditions, allergies, treatments or limitations and any change in the state of health.
The facility's responsibility for supervising the minor begins on entry to the premises for the activity and ends when the activity ends. The parent undertakes to bring and collect the minor at the times indicated, either personally or through persons delegated by them in writing. Before and after the activity, and during breaks outside the instructors' supervision, supervision is the responsibility of the parent.
The parent is aware that sporting activity, and in particular contact disciplines, horse riding and team sports, involves a risk of injury, and that such risks cannot be entirely eliminated despite compliance with precautionary rules.
The parent releases GYM Prova, its instructors and collaborators from all liability for damage to persons or property arising from the minor's imprudent behaviour or behaviour not in line with instructions, from undeclared health conditions and from breach of the rules, save in cases of wilful misconduct or gross negligence and rights that the law does not allow to be limited.
The facility is not liable for loss or theft of personal belongings and valuables left unattended in the changing rooms or on the premises.
The parent is responsible for the minor's behaviour and for any damage caused by the minor to persons, facilities and equipment. The facility may suspend the minor from the activity in the event of breaches of the rules.
In the event of injury or illness the facility may call the emergency services and notify the parent at the contact details provided, which the parent undertakes to keep up to date.
